Applying is a short, three-step process. Start it online below, and reach out any time if you have a question along the way.
Pay the application fee
Pay the $35 application fee online using the button below.
Complete the application
After you pay, you'll go straight to a page where you can download the application. Fill it out completely and sign it so we can review it without delays.
Return it to the office
Text, email, or drop off your completed application and documents at our office at 517 Main St, Beech Grove, IN 46107 (details below). We'll follow up with next steps.
Application documents
Along with your application, we'll need two documents:
- A recent pay stub
- A photo ID
You can text them to 317-513-4956, or email them to dbrackenridge@yahoo.com. A clear photo from your phone or a digital copy is perfectly fine. You're also welcome to bring them to our office at 517 Main St, Beech Grove during business hours, Monday, Tuesday, Thursday 9am-5pm.
